Use case: 

Patient E., 56 years old, suffers from arterial hypertension (BP 180/100 – 150/90 mmHg). Blood pressure began to rise at the age of 50 with the development of postmenopause. Over the past five years, I have gained 20 kg in weight.

• Objectively: increased nutrition. Height 156 cm, weight 90 kg. The body mass index is 40 kg/m2.  She has abdominal obesity. Waist size 103 cm, hip size 108 cm. The condition is satisfactory. The skin is clean. There is no swelling. In the lungs, breathing is vesicular, there are no wheezes. The heart tones are muted, the rhythm is correct. Heart rate 76 per minute The belly is soft, painless. The liver is at the costal edge.

• Additionally: 

total cholesterol 8 mmol/l; 

triglycerides 2.5 mmol/L; 

HDL cholesterol 0.9 mmol/L; 

fasting blood glucose 6.1 mmol/l; 2 hours after taking 75 g of glucose 10.0 mmol/L.

 Microalbuminuria: 250 mg/day.

 ECHOCARDIOGRAPHY: no valvular pathology was detected, the heart cavities were not expanded, the left ventricular myocardial mass index was 121 g/m2, signs of diastolic dysfunction, PV 62%.
