Topic outline

  • GENERAL

    We welcome you to the course on nutrition in COPD. This Moodle will introduce you to the basics of nutritional strategy in patients with COPD, and you will learn about the main goals of treatment therapy in this category of patients. The course will introduce you to the key aspects of nutrition for patients with COPD, macronutrients, and micronutrients necessary to normalize the status of patients.
    COPD is one of the non-communicable diseases and also the  and the number of patients with this disease is growing steadily every year. If medical professionals have the skills to correct syndromes that develop with this pathology using therapeutic nutrition and diet, a medical professional can improve the quality of life of patients, reduce the number of exacerbations of the disease, and increase life expectancy.
    Goals of the course:
    The goal of the course is to obtain the necessary knowledge to determine nutritional support recommendations for patients with COPD in accordance with specific medical needs in clinical practice.
    After the course, students should be able to:
    ✔ describe the guidelines of nutritional therapy for COPD;
    ✔ describe adaptable nutrition plans to meet specific medical needs based on the preferences of patients with COPD;
    ✔ explain and calculate recommended daily intake levels for macronutrients (proteins, carbohydrates, fats) and microelements (vitamins, minerals) in COPD patients, considering factors such as age, disease severity, and comorbidities;
    ✔ have skills to develop a nutrition recommendation to correct excess/deficit weight, muscle atrophy, and energy deficiency in COPD patients and to determine the indications, priorities, and effectiveness of oral and enteral nutrition for patients with COPD.

  • LESSON 1. Dietary Strategies for COPD and Nutritional Supplements in COPD.Weight Management in COPD

  • LESSON 2. The role of macronutrients and micronutrients in COPD

    In COPD dietary factors may modulate the impact of adverse environmental exposures or genetic predisposition on the lung, but can also have direct (protective or harmful) effects on the biological processes involved in lung function, disease development, and outcomes. The impact exerted by early-life and cumulative dietary choices on later-life health has been increasingly recognized for respiratory diseases, thus offering a greater window of opportunity for disease prevention. Furthermore, the abnormal nutritional status observed in advanced COPD patients, with unintended weight loss, muscle loss, low fat, and fat-free mass associated with the presence of emphysema is a recognized independent determinant of COPD outcomes and provides targets for nutritional interventions.
    7 hours are allocated to attend this lesson, to familiarize with the content and complete assignments, 4 of which are allocated for self-preparation.
    After this lesson you will be able to:

    1. Recognize the importance of the micro- and macro-nutrients in the management of COPD and its impact on the disease outcome and quality of Life.
    2. Define macronutrients (carbohydrates, proteins, and fats) and micronutrients (vitamins and minerals), and explain their distinct roles in maintaining health and supporting individuals with COPD.
    3. Evaluate the importance of micronutrients in immune function, antioxidant defenses, and other critical processes that impact the health of individuals with COPD.
    4. Identify common nutritional deficiencies in COPD patients, including vitamin D, vitamin C, and certain minerals, and understand their implications.
    To understand what Macronutrients and Micronutrients are and their roles, watch the following video:

    about macronutrients

    and

    about micronutrients

    Vitamins play an important role for the body. Watch the following video, which will allow you to find out what types of vitamins are and their functions, what deficiency conditions can develop when the vitamin balance is disturbed.

    COPD is a public health problem worldwide. Patients with COPD often have increased energy and protein requirements and decreased energy and protein intake. Adequate intake is necessary to improve lung function and immune system, prevent weight loss and maintain muscle mass and strength. In Lesson 1, you watched a video about energy and protein intake and its relationship to lung function in patients with COPD. If necessary, you can return to lesson 1 and watch this video again.

    Please study the presentation below "The role of macronutrients and micronutrients in COPD".

  • LESSON 3. Correction of therapeutic nutrition and diet in comorbidity of COPD with arterial hypertension.

    The most frequent COPD comorbidities were: hypertension (range, 17%–64.7%), coronary artery disease (19.9%–47.8%), diabetes (10.2%–45%), osteoarthritis (18%–43.8%), psychiatric conditions (12.1%–33%), and asthma (14.7%–32.5%). Several comorbidities had an impact on the frequency and severity of COPD exacerbations, quality of life, and mortality risk, in particular malignancies, coronary artery disease, chronic heart failure, and cardiac arrhythmias. Comorbidities, especially cardiovascular diseases and diabetes, are frequent in COPD patients, and some of them are associated with higher mortality.
    The presence of comorbid conditions in COPD changes both the symptoms and course of the disease, and should also make changes in the management of the patient, his drug and non-drug treatment, and of course should be taken into account in the prescription of therapeutic nutrition and diet.
    6 hours are allocated to attend this lesson, to familiarize with the content and complete assignments, 3 of which are allocated for self-preparation.

    After this lesson you will be able to:
    1.  Explain how does the course of COPD change in the presence of comorbid arterial hypertension.
    2.  Define changes in metabolism need correction in case of comorbidity of COPD with arterial hypertension.
    3.  Describe the parts of the nutritional therapy and diet of a COPD patient need to be additionally changed in case of comorbidity with arterial hypertension.
    Please, first review your knowledge about COPD, its pathogenesis, metabolic changes:
    watch the following video .
    This video will help you understand the metabolic changes in COPD  metabolism changes in COPD
    and
    this video provides excellent nutritional recommendations for patients with COPD nutritional recommendations
    Since in this lesson we are talking about comorbidity, that is, a combination of two diseases in a patient, check out this video, which well explains the pathogenesis of arterial hypertension
    and
    recommendations for nutrition for patients with arterial hypertension, note what differences these recommendations have with the recommendations for COPD, which you viewed above recommendations

    Please study the presentation below about key points of nutrition of patients COPD+AH.

  • LESSON 4. Patient Education and Counseling. Effective Communication with COPD.

    A key objective for patients with COPD is the prevention of exacerbations and hospital readmissions. Educating and counseling patients and their caregivers about their disease state and symptom management strategies plays a critical role in achieving this objective. Patient education relates to prevention, maintenance therapy or treatment of exacerbations.
    5 hours are allocated to attend this lesson, to familiarize with the content and complete assignments, 2 of which are allocated for self-preparation.
    After learning the lesson, the student should be able to:
    • Explain of the critical role of patient education in the management of Chronic Obstructive Pulmonary Disease (COPD)
    • Explain  how it contributes to improving patient outcomes and self-management.
    • Be able to create personalized educational plans that address each patient's unique needs and challenges in managing COPD.

    Watch this video and learn why nutrition education is important for patients with COPD

    then watch this video for quick, easy-to-follow nutritional advice for COPD patients

    You should know that patient education and self-monitoring at home is very important for this disease

    When teaching patients with COPD about nutrition, it is very important to have simple and understandable explanations, simple recommendations that can be easily followed at home, ready-made recipes and, of course, understandable videos for patients

    A good method is for the doctor to answer frequently asked questions by patients, topics that are really important for patients for self-monitoring the disease through nutrition at home
